Blood Tests

for Medication Monitoring

If you are currently taking any of the following medications we need to check your U & E’s, FBC and LFTs every year to ensure you are on the correct dose.

Amiodarone
Apixiban
Rivaroxaban
Azathioprine
Ciclosporin / Neoral / Ikervis
Lefunomide
Lithium / Pridal
Spironolactone / Eplerenone
Warfrain
Sulfasalazine
Mercaptopurine
Methotrexate
Penicillamine

Please contact the surgery on 0330 053 6456 after Midday to book an HCA appointment.

Please state you are booking for Medication Monitoring bloods and bring any letter received with you to your appointment alongside the Blood form.

All other routine blood tests

All other routine blood tests are taken by the Community Phlebotomy Team at Shrewsbury and Telford NHS Trust.

Please contact the Hospital on 01743 492510 to book a blood test. Patients will either be sent the appropriate blood form or asked to collect from the surgery. Please take the blood form enclosed with you. The hospital clinics are open Monday to Friday between the hours of 9.00am – 4.00pm.

We would like to stress that this is for your safety; while it is important that you take your medication regularly, we hope you will understand that we are responsible for ensuring that the drugs we issue do no harm and we cannot do this without your active participation in safe monitoring.

The practice will contact you only if the blood test is abnormal.
